THE THERAPEUTIC PROTOCOL OF DYSFUNCTIONAL SYNDROME OF TEMPOROMANDIBULAR JOINT

Abstract

Temporomandibular joint (TMJ) is the only mobile joint of the head that link the jaw to the skull. The jaw is part of the dento-maxillary system, an assembly of organs that work together to fulfill some important functions like: oral digestion, phonation, swallowing and reconstitution of human physiognomy.

            The dysfunctional syndrome of TMJ, known as temporomandibular dysfunction, is the term used for describing a group of signs and symptoms from the mobilizer muscles of the jaw, the temporomandibular joint and the associated structures.

            Although the TMJ disorders are not so often debated or found in medical practice, it was proved a global prevalence of 15-20% in adults. This syndrome has a prevalence of 1.5 and twice higher at females, the maximum prevalence is at fertile age of 20 - 45 years old.

            According to World Health Organization, one of ten people develops a painful condition that persists at this level 12 months, from which 50% have a problem and they don’t recover in the next 12 months.

            From this reason it’s very necessary to inform the population about the prophylaxis of different disorders that may occur at the temporomandibular joint, because it’s easier to prevent than treating the secondary symptoms of this dysfunctions.

            The essay presents a therapeutic protocol of the dysfunctional syndrome of the temporomandibular joint.

            Rezumat

            Articulația temporo-mandibulară este singura articulație mobilă a capului care leagă mandibula de baza craniului. Ea face parte din aparatul dento-maxilar, un ansamblu de organe care conlucrează la îndeplinirea unor funcții importante precum digestia bucala, fonația, deglutiția și constituirea fizionomiei umane. [1]

            Sindromul disfuncțional al ATM, cunoscut și sub numele de disfuncție temporo-mandibulară, este termenul folosit pentru a descrie un grup de semne și simptome de la nivelul mușchilor mobilizatori ai mandibulei, articulației temporo-mandibulare și a structurilor asociate. [2]

            Deși aceste tulburări de la nivelul articulației temporo-mandibulare nu sunt des dezbătute sau întâlnite în practica medicală, s-a constatat faptul că prezintă o prevalență mondială de 15-20% la adulți. Acest sindrom este de 1,5-2 ori mai prevalent la femei, prevalența maxima fiind la vârsta fertilă de 20-45 de ani. [3]

            Conform Organizației Mondiale a Sănătății, 1 din 10 pacienți dezvoltă o condiție dureroasă persistentă la acest nivel timp de 12 luni, din care aproximativ 50% nu âși revin în urmatoarele 12 luni. [4]

            Din acest motiv este foarte importantă informarea populației în ceea ce privește profilaxia diferitelor tulburări ce pot surveni la nivelul articulației temporo-mandibulare, întru cât este mult mai ușor de a preveni decât a trata simptomatologia secundară acestor disfuncții.

            Referatul prezintă protocolul terapeutic în sindromul disfuncțional al articulației temporo-mandibulare.
